Why do I get multiple errors when trying to upload my product database in my 5.0 store? (dbmake.cgi patch)

Product: SC + TX
Version: 5.0, 5.0.0.1
Platform: Linux, Solaris/Sparc, FreeBSD, and BSDI

When I try to upload my product database through Utilities > Database > Upload / Download, it doesn't complete. I receive various error messages. Examples:

".. There is an incorrect numeric value or format in one of your fields. Please check for invalid characters and misplaced punctuation and try again. This record was NOT created."

"...Unique field identifier [SKU] requires a value! This record was NOT created."

Product uploads may have problems when the Unique product identifier is SKU or the Name field is used but it is not the first field in the data to be uploaded. Symptoms include records being rejected during the upload, invalid data upload errors, and data from one field ending up in another field.
A work-around is to edit the database so that the name field is first. Then select Name as the Unique product identifier in the Upload Product Information screen.
