"Unable to load themes (themes.dat)." error when attempting an upgrade

There was a known issue with 4.2 upgrades that caused the themes.dat file to exist in two directories - data and ss. To fix, rename the themes.dat file to themes.dat.old in the data directory and make sure a copy of any customized themes.dat file exists in the upgraded ss directory.
This has been fixed in subsequent releases.
